New Revenue Agreement Form

Site Directors and Executive Directors:  Please use the following form to request the creation of a new or amended revenue agreement. 

Required if this a new client. 
The person to whom the draft agreement should be sent for approval. 
Please note: due to the time required to have a new revenue agreement negotiated and approved by UC Davis Business & Revenue Contracts, agreement requests must be submitted at least a month before work is scheduled to commence. No work can take place until an agreement is in place.

Hourly rate is determined by your project's approved rate agreement. Charging an amount other than the approved rate for the same type of services is not permitted. If you have any questions regarding how to charge appropriately, please contact Jeremy Till

A scope of work is required to initiate a new revenue agreement. If the client requires other forms to be filled out and/or approved with the agreement, please upload them here as well.
