Independent Contractor Form

CSMP Site Directors and Statewide Office Executive Directors: Please use this form to request the establishment of a new agreement with an independent contractor (such as a teacher leader or other partner) who will be performing work on an ongoing basis over an extended period of time. For individuals who are presenting or participating at an individual event, please use the one-time payment form. If you have questions about which form to use, please contact Jeremy Till.

Contractors cannot begin work until an agreement is in place. Once an agreement is in place with an contractor, they can receive payment by submitting invoices that reference the relevant UC Davis Purchase Order (PO) number.

Note that current UC employees (at UC Davis or any other UC campus) cannot be be paid as independent contractors. 

Due to the time it takes for agreements to be approved by UC Davis Purchasing, please submit request at least two weeks before work is to commence. Work cannot commence until agreement is in place.
